An internal NYPD disciplinary case opened on 4/4/2013 alleged that this officer engaged in the following conduct: 1) Failed To Make Timely, Regular And Consistent Entries To His Activity Log - Guilty. 2) Caused Inaccurate Information To Be Submitted To The Department In That He Prepared And Submitted A Police Officer's Monthly Conditions Impact Measurement Report With Information Concerning His Enforcement Activity That He Knew Or Should Have Known Was Inaccurate Or Erroneous - Guilty. 3) Failed To Consistently Sign Himself Out On The Return Roll Call - Guilty. The officer received the following penalty: Forfeiture Of Eight (8) Vacation Days.
